Hypnosis Client Contract & Bill of Rights
Confidentiality: All hypnosis sessions are confidential. Karen Kenney LLC will not release any information to anyone without written consent from you, the client, except as provided by the law.
Education and Training: I am a Certified Hypnotist and a certified Integrative Hypnotist, a certified Integrative Change Worker and Life Coach. I'm a member of the National Guild of Hypnotists, IACT and The Ethical Coaching Collective. I am also a Certified Spiritual Mentor and Certified Yoga Teacher. I participate in continuing education to learn new modalities and to maintain my professional training at a high level.
I am not a physician nor a licensed health care provider and may not provide medical diagnosis nor recommend discontinuance of medically prescribed treatments. I am not a therapist or a licensed mental health counselor. A client has a right to refuse hypnotism services at any time. A client has a right to be free of physical, verbal, or sexual abuse. A client has a right to know the expected duration of sessions and may assert any right without retaliation.
In my practice, I use hypnosis to help people discover the subconscious reasons for their symptoms, behaviors, or habits. When they are discovered, I use tools to help the client release them and relearn a new positive way of thinking. My work combines a number of powerful and effective techniques. We work with both the subconscious and conscious processes so that deep changes are taking place, but you also have practical strategies to support and empower you. The more committed and consistent you are, the more effective and lasting the transformation will be.
Fees and Cancellation Policy: $250.00 - Initial Consultation
* Scheduled sessions are reserved exclusively for you. If you need to reschedule or cancel an appointment with me, please contact me within 48 hours of the appointment. There are no refunds. Any unused sessions in this program are non-refundable.
Redress: I offer hypnosis services according to the guidelines and ethics prescribed by the National Guild of Hypnotists. Feel free to contact the National Guild of Hypnotists at www.ngh.com with any questions you may have regarding my services.
Client Consent and Release:
I am of legal age and in consideration of my acceptance as a participant in hypnosis and hypnotherapy sessions, training, seminar or any other service, activity, or event conducted by Karen Kenney LLC, releases her from all claims and damages, copyright, demand or actions whatsoever in any manner arising from my participation. Further, I understand that should audio recordings be made of my sessions, Karen Kenney LLC, retains the copyright of these recordings and will not release them without my written consent.
I declare that I have read the above document and that I fully understand and agree to the terms described. I acknowledge receipt of a copy of this statement.
